• Palliative Care Practitioner Pathway (Tier 1 and Tier 2)

• Grief Literacy and Educator Pathway (Track 1 and Track 2)

• Bundled pathway options

The questions below help us understand your background, experience, and how we can best support your learning. 

How the application process works

The application process has two simple steps.

Step 1 - Complete this online application form
You will share some basic information about yourself, your experience, and the program you are applying for.

Step 2 - Complete the full questionnaire (sent by email)
After you submit this form, you will receive an email with a downloadable application questionnaire in PDF format. You can fill it in on your computer and email it back to us.

For Tier 2 and full palliative pathway applicants
Some applicants will also be invited to schedule a brief interview as part of the application process. Instructions will be sent to you by email if this applies to you. 

A few things to know before you begin 

• There are no right or wrong answers. We are looking for fit, readiness, and how to support you, not perfection.

• You are welcome to pause, reflect, and come back to the form if you need a moment.
